STANDARD HOSPITAL BENEFITS
In-Patient Services
Subject to provisions herein, the Standard Hospital Benefit shall consist of the following services:
- Accommodation and meals at the standard or public ward rate;
- Full nursing services;
- Laboratory, radiological and other diagnostic procedures, including biopsies (except that surgeon’s fees are not included), together with the necessary reports, for the purpose of maintaining health, preventing disease and assisting in the diagnosis and treatment of any injury, illness or disability;
- Drugs, biological and related preparations which are prescribed by an attending physician in accordance with the hospital formulary and administered in the hospital.
- Use of operating room, anesthetic facilities and other facilities required in operating procedure, including necessary equipment and supplies;
- Standard surgical supplies;
- Use of radiotherapy facilities;
- Use of physiotherapy facilities;
- Services rendered by persons who receive remuneration for that service from the hospital;
- Use of haemodialysis facilities and up to $30,000 towards the cost of a kidney transplant and thereafter the full cost of all maintenance drugs;
- Treatment for alcoholism (other than alcoholism causing acute mental illness);
- Use of ultrasound facilities;
- Diabetic education and counseling but limited only to one education and counseling programme;
- Hospice care in establishment under the charge and management of the Board or in an establishment approved by the Commission;
- Speech therapy (in-patient);
- Use of orthopaedic appliance;
- Hyperbaric and wound care treatment;
- Bone densitometry; and
- Cardiac care programme.
Special conditions apply (1) Maternity Benefit (2) Artificial Limbs and Appliances (3) Mental Illness, Alcohol and Drug Abuse which are set out.
Limitations of Benefit
Subject to the Hospital Insurance (Portability) Regulations 1971, benefit in respect of in-patient treatment shall apply without limit as to the duration of the period in hospital.
Out-Patient Services
- Pathological studies, X-ray and other diagnostic procedures not obtainable or generally provided in a doctor’s office as prescribed by a physician, including biopsies (except that surgeon’s fees are not included), together with the necessary reports, for the purpose of assisting in the diagnosis and treatment of an out-patient;
- The use of radiotherapy, occupational therapy and physiotherapy facilities in the hospital when prescribed by a physician;
- The hospital component of out-patient services necessary for the initial treatment of accidental injuries suffered within forty-eight hours preceding the time of treatment or of acute illness and the hospital component necessary to support operative or diagnostic procedures;
- Local ambulance services in essential cases,
- X-ray of the breast performed –
| (a) | at the general hospital; or |
| (b) | at such other facility, and at such rates as may be approved by the Commission; |
- Hospital services rendered to an out-patient by hospital staff outside the hospital for specific purposes;
- Speech therapy rendered to an out-patient provided that the patient had previously been an in-patient and that the treatment does not exceed three month duration;
- The use of haemodialysis facilities;
- The use of orthopaedic appliances;
- Asthma education;
- Hyperbaric and wound care treatment;
- Bone densitometry; and
- Cardiac care programme.
Maternity Treatment
In lieu of any other benefit provided in this contract, benefit will be payable for confinement as a result of childbirth, pregnancy or miscarriage in accordance with the Standard Hospital Benefit as stared in this contract, provided that the contract has been in force for a period of ten consecutive months immediately preceding such confinement.
Artificial Limbs
The contract includes the supply, maintenance, repair and renewal of an artificial limb, or any artificial appliance as deemed in the Hospital Insurance (Artificial Limbs and Appliances) Regulations 1971. The following shall apply: the liability of an insurer in respect of the supply of an artificial limb or artificial appliance shall be limited to maximum of $15,000; however, where the amount of the maximum liability can only be in respect of the maintenance, repair or renewal of any artificial limb, or artificial appliance and such liability shall be limited to 80% of the unexhausted portion of the $15,000.
Mental Illness, Alcohol, and Drug Abuse
The contract includes the following treatment at the Mental Hospital:
| (a) | In-patient treatment (including the cost of accommodation, meals, pursing and ancillary services), of acute cases of mental illness, including those caused by alcohol and drug abuse;
|
| (b) | Out-patient treatment in respect of desensitization injections for cases of alcohol and drug abuse and other psychotic conditions, electroconvulsive therapy and electroencephalograms. |
The following in-patient treatment does not apply-
| (a) | To treatment for any period in excess of 40 days in any calendar year |
Portability
Expenses incurred in an approved hospital outside Bermuda may be recovered subject to the terms of the Hospital Insurance (Portability Regulations, however:
| (a) | in respect of in-patient treatment of a particular disability, be limited to expenses incurred over a period of not more then 45 days during a twelve month period;
|
| (b) | the amount recoverable shall not exceed the amount which would have been payable at the applicable public ward per diem rate if the treatment had been received in the general hospital. |
Exclusions under the Regulations are as follows:
| (a) | the cost of transportation to or from the approved hospital; |
| (b) | the cost of any treatment for alcoholism; |
| (c) | the cost of any hospice care. |
Exclusions
The Standard Hospital Benefit does not include the following:
- Treatment of mental disorder, nervous disorders (other than those with a defined pathological cause), chronic alcoholism or drug addiction except treatment prescribed in the contract under the section Mental Illness, Alcohol and Drug Abuse.
- Rest cures, sanitaria and custodial care including in-patient in geriatric and rehabilitation ward of the general hospital;
- Cosmetic or plastic surgery unless necessary to correct traumatic injury;
- General health examination, dental work or treatment, dental X-ray, extractions, fillings and general dental care except dental surgery for the excision of impacted teeth or of a tumor or cyst or treatment of sound natural teeth damaged as a result of an injury;
- Treatment involving examination of the eye or ear for the purpose of fitting eye glasses or hearing aid except where such a treatment is necessitated by damage to the natural eye or ear as a result of injury;
- The provision of medications for the patient to take out of the hospital;
- Diagnostic services performed to satisfy the requirements of third parties;
- Visits solely for the administration of drugs, vaccines, sera or biological products;
- Transportation or travel other than local ambulance services;
- Treatment or advice in the out-patients or emergency department which would normally be provided in a doctor’s office (such treatment, however, will be provided if it can be shown that the patient’s doctor was not available or the condition which is considered to be an emergency arose at a time when the doctor’s office is normally closed. For the purpose of this paragraph ‘emergency’ means the sudden occurrence of a physical illness requiring professional assessment and care);
- Treatment given or hospital facilities used which have not been prescribed by a registered practitioner, unless such treatment or use is certified as urgent and necessary by a medical officer employed by the Board.
Application to General Hospital
In relation to treatment provided in the general hospital, standard hospital benefit shall include services and supplies which are provided by the Board or its officers and servants, but shall not include fees for professional services provided in a private capacity or provided by any physician, surgeon, dental practitioner or other practitioner of a profession associated with medicine who is not an officer or servant of the Board.
